Real Choice Systems Change grant

In April 2003, COVE received a portion of the Real Choice System Change grant from the Center for Medicaid and Medicare Services to establish the Vermont Association of Professional Care Providers (VAPCP).  The objective is to create a stable, valued, and adequately reimbursed workforce to provide quality of care for individuals and families.

 

Consumers continue to experience a lack of choice and control over their service options.  The goals of the Real Choice Systems Change Project are to effect enduring systems that promote continued progress toward community integration of services, and promote real choice about how, where and by whom services and supports are delivered.

 

Three systems are partners in Real Choice Systems Change grant and have evolved separately over time.  The Vermont Department of Aging and Disabilities, the Vermont Division of Developmental Disabilities and the Vermont Division of Mental Health are working collaboratively to increase community integration, real choice and control for elders, younger adults with physical disabilities, people with developmental disabilities and their families, and adults with severe mental illness.
